IMPORTANT DIRECTIONS FOR REGISTRATION - PLEASE READ
CNY and FM Pop Warner are local affiliates of the Pop Warner Little Scholars, which set the standards and requirements. The registration is setup to abide by these standards by completing and submitting the application during the registration process. Please read the instructions and download the available set forms below. Print, complete, and upload within the online registration.
The application must be complete and meet the standard quality requirements.
Please utilize the helpful checklist below. They are required.
1. Completed Online Registration and Payment (completed at checkout)
2. 2023 participant contract and parental consent form
- Uploaded at Registration AND brought to Equipment pickup night.
3. 2023 physical fitness and medical history form (must be completed in 2023 and signed by Dr.) Or, A physical dated in 2023 also signed by a doctor.
- Uploaded at Registration AND brought to Equipment pickup night.
4. A Clear Copy of birth certificate
- Uploaded at Registration AND brought to Equipment pickup night.
5. 2" by 2" current color photo of player (must be in color, a headshot and be 2x2 inches in size

6. 2022-2023 report card (must be a photocopy of the complete report card, including all quarters, teacher comments, front and back of report card; must be readable)

7. FM Pop Warner Participant Code of Conduct (online accept at registration)
8. FM Mandatory Volunteer Contract (online accept at registration)
9. Emergency Medical Authorization (online accept at registration)

Other notes on registration and programs
FM Pop Warner is affiliated with Pop Warner Little Scholars. We operate within the local CNY Pop Warner jurisdiction.
We offer age-based divisions. Programs/divisions are dependent on meeting minimum player enrollment levels and staff to activate a program. We at FM Pop Warner work to ensure a safe and successful program for all participants. Should a program and/or division not meet the minimum enrollment standards, we will contact you directly to offer alternative options.

If you are from an outside district, please check with your local pop warner on waivers to play for another league. Send waiver to [email protected] along with your registration.
